The severity of ED has been correlated with the extent of CVD. Phosphodiesterase inhibitor medications, such as sildenafil (Viagra) can cause a reduction in blood pressure, and so should not be taken by patients known to have low blood pressure (hypotension), or in those who have recently had a stroke or heart attack. Similarly, they should not be used in patients who take nitrate medication (such as glyceryl trinitrate, GTN, or isosorbide mononitrate) for chest pain. Phosphodiesterase inhibitor medications are associated with some side-effects, which include headaches, hot flushes, and dizziness. However, placement of a prosthesis causes scarring of the tissue within the corpora cavernosa, and if the prosthesis requires removal, other forms of therapy, except for the vacuum device, are often not effective. Thus, most physicians reserve placement of a prosthesis for men who have tried and failed or have contraindications to other therapies.
A duplex ultrasound is a diagnostic technique that uses painless, high frequency sound waves to measure blood flow in structures such as the penis.The procedure itself is painless. It is usually performed before and after injection of a smooth muscle relaxing medication into the penis, which should significantly increase the diameter of the penile arteries.A duplex ultrasound is most useful in evaluating possible penile arterial disorders. Further studies of the venous drainage system as well as arterial imaging are usually recommended if vascular reconstructive surgery is anticipated.
